These programs provide support to older (55+) adults who currently lack the personal resources to maintain their health and well-being in the community. Care management is provided by experienced social workers, nurses and therapists that direct the provision of necessary services for qualified seniors.

Independence at Home’s Goals:

  • Keeping older adults and their caregivers healthy and independent by:
    • Providing direct services to help older adults live safely in the community
    • Building access to programs and services vital for physical, emotional and social wellbeing
    • Invest in the community through support of community based organizations and partnering with city, county and statewide efforts in key areas impacting older adults.
